Students with funding from the California Department of Rehabilitation (DoR) may use these funds to enroll in courses with UCLA Extension. A completed Authorization and Invoice for Direct Service Fees form is required from a student's designated counselor at the DoR in order to process enrollment.
Completing the Authorization Form
The authorization must be issued to UCLA Extension, 1145 Gayley Ave, Los Angeles. DoR counselors must select the correct vendor profile. The authorization must also include the following:
- Seven-digit student ID number - Students who do not already have a student profile with UCLA Extension may create one via the UCLA Extension student web portal.
- All course information including the six-digit course section or REG number - The course REG number is especially important as it specifies the exact course section the student wishes to enroll in. The student should pay special attention to the course delivery format, lecture/meeting days and times, and instructor. The course REG numbers and course fees are posted on our website shortly before the start of open enrollment for the quarter. Refer to the UCLA Extension academic calendar for open enrollment dates. All relevant course information can be found on the individual course pages on our website.
- The Dates of Service indicated on the authorization must cover all published lecture/meeting dates as indicated on the individual course pages on our website.
- The specific name of the Certificate or Candidacy Fee in the authorization when a student intends to complete a certificate program.
Incorrect or incomplete authorization forms will be sent back for correction. Submit completed authorization forms to billing@unex.ucla.edu.
Course Transfers
In the event a student decides to transfer to a different course section, UCLA Extension will require an amended authorization from the DoR which reflects the new course information.
Processing Time
Please allow 1 to 2 business days to review and process completed authorization forms. It is strongly recommended that approved authorization forms be submitted to UCLA Extension no later than 1 week prior to the start of the academic quarter to avoid delays and to help ensure students are able to enroll in the course section(s) of their choice.
