The Systems Access Request (SAR) form, when approved by an individual's supervisor, authorizes specific access and transaction limits associated with various UCLA campus systems, functions, and FAUs. This article briefly outlines how to submit a new or revised SAR.
Systems Access Request (SAR)
The Systems Access Request or SAR form is used to request access to various campus systems and functions. Staff members and their supervisors complete each SAR, in consultation with their BFS support team, based on the specific role and work duties assigned.
Preparing the SAR
Click here to download the current Extension SAR form. Please contact your BFS support team for assistance completing the form, particularly if you are uncertain what types of access to request.
Completed and signed SARs should be submitted to the Extension DSAs for processing. Most requests can be processed within 1 to 2 business days of submission. Certain access may require DSAs to submit separate requests to campus which may take longer to process.
